Cook; his children, Vanessa Roberts, Richard Cook (Samantha), Michael Burns (Rachael), Ryan Burns (Brandy), Kevin Burns (Kristen) and Robert Burns (Misty); his grandchildren, Xavier Roberts, Lily Cook, Anthony Burns (Inez), Tori Burns, Hailey Russell (Jake), Ryan Burns, Brendan Burns, Kellen Burns, Jenna Burns, Kaitlyn Burns and Jack Burns; and his sister Marlyn R. Cook.
In lieu of flowers, contributions in his memory are encouraged to either the Hospice of the Western Reserve or a charity of the donor's choice.
