He was predeceased by: his parents, Bert Neal and Nora Evelyn Collinsworth; and his brother Rick Collinsworth. He is survived by: his wife Kathy; his children, Neal Collinsworth (Brandy Kumpf), Adam Collinsworth, Craig Collinsworth (Bridgett), Keith Collinsworth (Brenda), Amy Cox (Chris), Kristin Joslin (Scott) and Melissa Snyder; his grandchildren, Luke, Karlissa, Sofia, Stella, Izavea, Mateo, Kelsie, Cole, Olivia, Morgan, Ava and Brody; and his sister Pat Minella (Ray). He is also survived by many nieces and nephews.
