He was predeceased by: his parents, Richard A. McCullough and Consuelo B. McCullough (Moog); and his siblings, Linda Hartsock, Rick and Jason McCullough.

He is survived by: his siblings, Pat McCullough (Suzi) of Montpelier, Ohio and Sandy Jones of West Unity, Ohio. He is also survived by numerous nieces and nephews.

Memorials are requested to the charity of the donor's choice.
