He was predeceased by: his parents, Hershel Collum and Opal Collum; and his siblings, Tommy, Kenny, Roger and Shirley.
He is survived by: his siblings, Nora (Gary), Marlene and Leonard; and his children, Steven, Jackie, Chouna, Shannon (John), Missy, Billy, Jack Jr. (Johnna), Tommy (Amy), Slena (RC), David (Lori) and Terry. He is also survived by 20 grandchildren; 7 great grandchildren; and many nieces, nephews, cousins, and friends.
