He was predeceased by: his wife Madeline (Dory); his daughter Sherry; his sisters, Willa Jo Wells (Bill) and Ellen Ames (Gary); and his parents, Pearl Cook and Crawford Cook.
He is survived by: his sister Violet "Pauline" Martin; and his children, Debbie Buck (Jim), Terri Cook and Mark Cook (Cyndy). He is also survived by six grandchildren and five great-grandchildren.
