He is survived by: his wife Susie; his son Jonathon Carmichael; his grandson Bailey Carmichael; his mother Fay Kelly; his siblings, Michele Hensley and David Kelly; his nephew Jacob Hensley (Amanda); his great-niece Maci; his siblings-in-law, Tom Cusma (Faye), John Cusma and Pat Cusma; his aunt Jeanette Clark; his cousins, Phil, Joe (Kim) and Gary Clark; his grandson Jonathon; and his sons, Gustavo, Flavio and Juliano.
