He was predeceased by: his parents, Curtis Bolen and Bonnie Bolen (Conley); his daughter Amber Rachelle Bolen; and his nephew Blaine Morris. He is survived by: his former wife Yolanda Bolen; his significant other Paula Barkley; his sons, Travis Bolen of Ada and Chad Bolen (Gwen Gibson) of Ada; his grandson Tanner Keith Bolen of Ada; his siblings, Chris Bolen (Jean) of Columbus, Kirby Bolen (Kathy) of Cincinnati, Mike Bolen (Marie) of California, Curtis Bolen (Michelle) of Florida, Douglas Bolen of McGuffey and Cathy Duprey (Howard) of Alger; and his friends, Perry Caudill of Lakeview and Grover VanHoose of Fostoria.
