He is survived by: his wife Rebecca Adkins Cook; his children, Jimmy Dunn of Mebane, Kevin Dunn of Mebane, Samantha Bondurant of Charlotte, Michaela Cook of Boston, Cameron Cook of Greenup, Connor Adkins of Ripley, Katherine Smith of Ripley and Ian Cook of Ripley, Ohio; and his sisters, Carrolleen Putis of Kansas City, Rene Long and CJ.
