He was predeceased by: his parents, Andrew Kozak and Alexandria Kozak (Dziekowinski); his brother Andrew; his sister-in-law Virginia Kozak; his sister Cynthia; and his siblings-in-law, Stephen Vance and Rachael Ciruzzi.

He is survived by: his wife Linda A. Paciello; his children, Jamie Kozak-Williams, Allison Cacciotti (Angelo) and Westmoreland; his grandchildren, Shauna, Anjalie, Gennaro, Harper Rae and Paul Ciruzzi; and his son-in-law Angelo. He is also survived by many nieces and nephews, including special nephew.
