He was predeceased by: his sister Judy. He is survived by: his wife Carol Ann Puckett; his children, Ellen Puckett, Thomas Puckett (Ann) and Steven Puckett (Tracy); his grandchildren, Arianna, Vivian and Quinn Puckett; and his siblings, Paul, Carol and Barbara. He is also survived by several nieces and nephews.
