He was predeceased by: his father Gary T. Lezotte; and his brother Gary Thomas Lezotte. He is survived by: his parent Gail Gorman Lezotte; his brother Kenneth P. Lezotte; his cousin Cheryl Thibodeau; and his goddaughter Nicole Johnson. He is also survived by several other cousins and many loving and close friends.
