He was predeceased by: his parents, Alfred J. Szurek and Florence E. Szurek (Malicki); and his sister Sandra Perillo. He is survived by: his wife Sheila A. Oakes Abare; his children, Dan and Sarah of Gainesville, FL; his brother-in-law Tom Perillo; his nieces and nephews, Thomas Perillo, Jr. and Natalie Gonzales; and his caregivers, Heather, Nancy, Katrina, Tracy, Tammy, Ashlynn, Roxanne, Nicole, Christy and Michelle. He is also survived by one aunt and several cousins.
