Lynn Grace Polakowski Obituary

We bid farewell to Lynn Grace Polakowski (Wappingers Falls, New York), whose journey of life gracefully concluded on February 3, 2025 at the age of 78. Lynn Grace touched countless lives with warmth and kindness, leaving behind cherished memories that will forever echo in hearts. She was loved and cherished by many people including: her husband Tom; her children, Lori Jones (John), Kris Clayton (Jeff) and Tommy Polakowski (Mandy); her grandchildren, Bobby (Samantha), Zach (Ashley), Jacob, Jared, Jenna, Cora and Tiggy; her great grandchild Carlo; her parents, Jack VanVoorhis and Ginny VanVoorhis; and her siblings, John, Robert, Karen, Raymond, Patrick and Michael. She was also cherished by many cousins, nephews, and nieces as well as many, many friends.

Visitation was held on Friday, February 7th 2025 from 4:00 PM to 7:00 PM at the Delehanty Funeral Home (64 E Main St, Wappingers Falls, NY 12590). A funeral Mass was held on Saturday, February 8th 2025 at 10:00 AM at the St. Martin de Porres Parish (118 Cedar Valley Rd, Poughkeepsie, NY 12603).

In lieu of flowers, Lynn's family requests donations to the Cardinal Hayes Home for Children, PO Box CH, 60 St. Joseph Drive Millbrook, NY 12545 in Lynn's honor.
