He is survived by: his father Chester Panek; his significant other Michele Hansen; his children, Tim (Christi), Chris and Jaime (Chris); the mother of his children Debbie Panek; his grandchildren, Joshua, Matthew, David, Abigail, Riley and Reese; his siblings, Julie (John), Tom (Susan), Tim (Deary), Jim, Mike (Wendy), Barbara (Jerry) and Steve; and his aunt Ginny (Ron). He is also survived by many cousins, nieces and nephews.
In lieu of flowers the family requests that you play a round of golf in John's memory, or make a donation to the Michael J. Fox foundation in his name.
