He was predeceased by: his parent Fred Bernard; his son James Bernard; and his brother Fred. He is survived by: his parent Ada Edwards Bernard; his wife Esther Haun Bernard; his children, John F. Bernard Jr. of Walden, Jacqueline Rose (Daniel) of Pine Bush, Joann Franz of Wallkill and Janice Kennedy (Frank) of Walden; his sister Alice Gillotti of Walden; and his grandchildren, Toby, Tabitha, Shannon, Suzette, John III, Jeniffer, Patrick, Jeremy, Matthew, Frankie and Emilee. He is also survived by 11 great-grandchildren; nieces and nephews.
Memorial contributions may be made in Jack's name to St. Jude's Children's Hospital or the charity of one's choice.
