He was predeceased by: his parents, William Cook and Pauline Cook; his daughter Michelle Cook Angel; his grandson Micah Angel; his sister Deloris Beckhorn; and his siblings-in-law, Robert Beckhorn, Ronald Campbell and Norma Jean Campbell.
He is survived by: his wife Barabara Cook; his grandchild Ashton Angel; his son-in-law Michael Angel; and his brothers-in-law, Arden Campbell and Robert Franklin. He is also survived by several nieces, nephews, and cousins.
