He was predeceased by: his parents, Gerald Cook and Elizabeth Cook (Durdan); his siblings, Robert and Elizabeth; and his siblings-in-law, Joan Cook, John Petronio and Daniel Vanleeuwen.
He is survived by: his sons, David Cook and William Cook; his granddaughters, Alexandra (Allie) and Gianna (Gigi); his girlfriend Janet Benjamin; his girlfriend's children, Judah Feingold (Suzy) and Miriam Feingold (John Hernandez); his cousin Mary Theresa Major; and also, Sabrina, Julien and Betty Loman. He is also survived by many nieces, nephews, grandnieces and grandnephews, cousins, close friends, coworkers.
In lieu of flowers, Fred requested donations be made in his memory to Hospice and Palliative Care (http://www.hospicecareinc.org) or Spring Farms Cares (http://www.springfarmcares.org), organizations that were close to his heart.
