He was predeceased by: his parents, William Lewis and Beulah LaPorte Lewis; his children, Phillip Lewis and Jessica Lewis; his son-in-law Chuck Leavitt; and his brother-in-law Jack Sarna.
He is survived by: his wife Dona; his children, Bill (Jill), Scott, Bob (Sue), Jeffrey, Heather Lewis Buchholz (Keith), Sharon Neeld Leavitt and Craig Neeld (Michele); and his siblings, Lynette Sarna and Michael (Donald). He is also survived by friends and relatives.
In lieu of flowers, the family suggests donations to Cure SMA, St. Jude Children's Research Hospital or the Ronald McDonald House.
