He is survived by: his parents, Donald Brent Cook and Sandra Borter Cook; his sons, Derrick Cook and Tyler Cook; his siblings, Brenda Drake (Rick), Laura Wallace (Dave) and Jamie Cook (Chris Galloway); and his grandmother Shirley Borter. He is also survived by many nieces and nephews.
