He is survived by: his sister Adele Pillitteri of Clovis, CA; his children, Martin Crawford of Tonawanda, NY and Jennifer Forth (James Day) of Lawrence, KS; and his grandsons, Declan Forth (QuyenTran) of Lenexa, KS and Logan Forth of Kansas City, MO.
