He is survived by: his daughter Michaela Mikulsky (Stephen Mikulsky); his grandchildren, Carolyn and Clark Mikulsky; and his siblings, Kathleen O'Toole, Michael O'Toole, Kim Eager, Kelly Nottingham (Eager), Kelly O'Toole-Scotto and Bridget Mayer.
