He was predeceased by: his parents, John Joseph Hayes Jr. and Gladys Whitehead Hayes. He is survived by: his sisters, Karen Hayes Gazda of Jacksonville, FL and Megan Hayes Fidgeon of Jacksonville, FL. He is also survived by several nieces and nephews.
