He was predeceased by: his parents, DeVer "Cookie" Cook and Bonnie Cook; and his stepfather Leslie Kirk. He is survived by: his wife Gwendolyn Cook; his children, Maddison, Mason, Madilynn, Alyvia and Annileece; his stepmother Christine Kirk; his siblings, Henry, Leon, Sam and Dawn Green; and his stepsiblings, Joseph Knapp, Shane Knapp (Melissa), Richard Kirk (Christy), Tanya Kirk (Luis), Marie Burns and Kevin Peyton. He is also survived by numerous nieces and nephews.
