He was predeceased by: his grandparents, Ed and Diane Godley (SugarMa). He is survived by: his parents, Douglas Slade (Jennifer) of Newark Valley and Sheri Lea Godley-Whitley of Spencer; his close friend Taylor Keturi; his son Tucker Douglas Slade; his brother Jon Leonard (Cortney) of Spencer; his nieces and nephews, Odin and Lucas; his sister Cherol Whitley of Spencer; his grandparents, David Slade of South Carolina and Lillian Slade of Spencer; his aunts and uncles, Robert Butcher, Deborah Fedenko (James), Deborah Briggs, Pamela Wellin (Jim) and Jen Butcher; and also, Travis Wylie, Julianna Ferwin, Fred Maki (Heather) of VanEtten and Shaun Keturi (Erica) of Odessa. He is also survived by many other friends, and extended family.
