He was predeceased by: his father Richard T. "Terry" Van Slyke Sr.. He is survived by: his close friend Sue Conger Sgarlata; his mother Patricia Jonas (Eugene); and his siblings, Daniel Bane, Michael Bane (Lisa), Patrick Bane (Maureen), Maureen Meyering (Rob) and Christopher Van Slyke. He is also survived by many nieces and nephews.
