We are sad to announce that on February 7, 2025 we had to say goodbye to Douglas Karl Forman (Setauket, New York). You can send your sympathy in the guestbook provided and share it with the family.

He is survived by: his children, Dina Forman, Suzanne Tomicic (Steve) and Douglas Forman (Katy); his grandchildren, Ava Tomicic, Brianna Tomicic, Lucas Forman and Christian Forman; his former wife JoAnn Forman; and his sisters, Corinne, Carol and Denise.

Visitation was held on Thursday, February 13th 2025 from 2:00 PM to 4:00 PM and from 7:00 PM to 9:00 PM at the Bryant Funeral Home (411 Old Town Rd, Setauket- East Setauket, NY 11733). A Mass was held on Friday, February 14th 2025 at 9:30 AM at the St. James RC Church (429 NY-25A, Setauket- East Setauket, NY 11733).

In lieu of flowers, the family requests donations be made to American Cancer Society.

“It is with the greatest of sadness that I have learned of the passing of Douglas Forman, one of my most valued officers while a Sergeant in SCPD Highway Patrol. Along with his partner John Newhoff, the two of them would often drive me nuts, only agreeing to meet at certain times. Thursday mornings were definitely out! Breakfast in Babylon!!! A real pair, but two of the finest officers I ever had the pleasure of knowing. RIP Doug, Thanks for your service and friendship, Insp John Triandafils”
