She was predeceased by: her father David Puckey. She is survived by: her mother Darlene Puckey; her son Zachary (Alissa); her close friend Karen LaRusso; her family, Gino and Dominick; her daughter Sabrina; her grandson Kayden; and her siblings, Andrew (Kristine), Sara (Jason) and Heather (Jimmy). She is also survived by nephews, nieces, cousins, aunts and uncles.
The family would like to offer deepest thanks to all of the nursing staff on the 6th floor Bone Marrow Transplant Unit at the Wilmot Cancer Center, whose kindness and compassion were crucial to Amy's well-being, especially during the pandemic which limited her physical contact with her family.
In lieu of flowers, we ask friends and family to instead support the Go Fund Me that has been established to help with final expenses, and to establish a college fund for Amy's grandchildren, as these were her wishes.
