He was predeceased by: his parents, Hubert Hazel and Katherine McCall; and his siblings, Marie Baine, Daniel McNeil and Clarence Perkins.
He is survived by: his wife Nancy J. Holt; his children, Freddie Toms Jr., Rodger Storey and Demetri Redd (Gilbert); his stepchildren, George Whalen and Kate Koch; and his siblings, Larry Hazel (Muriel), Hubert "Tiny" Hazel (Kim), Billy Hazel, Lester "Buster" Hazel (Martha) and Dorothy "Gina" Hazel. He is also survived by many nieces, nephews, grandchildren, and great grandchildren.
