He is survived by: his children, Tracy Collins Ballew (Jacob) of Dalton, GA, Dawn Wahl (Shawn Page) of Otto, NY, David Jr. Waite (Kim) of Little Valley, NY and Kyle Waite of Salamanca, NY; and his brother Daniel Waite of Salamanca, NY.
