He was predeceased by: his parents, Mary Kate Moffett Butler and Martin Michael Butler; and his siblings, Timothy, Marty, Margaret and Tommy.
He is survived by: his wife Theresa Fanella of Harrison; his children, Jacqueline Johnston (John) of Harrison, Brian Butler of Rye, Terese Bisconti (Robert) of Bronxville, Kathleen Brown (Curtis) of Carmel Valley, Patrick (Debra) of Ridgefield and Timothy (Christina) of South Salem; and his brother Kevin Butler (Barbara) of Rye. He is also survived by 14 grandchildren and six great-grandchildren.
In lieu of flowers, donations may be made in Jack's memory to Cardinal Hayes High School, 650 Grand Concourse, Bronx, New York 10451.
