He is survived by: his sons, Donald Kazmark III of Blue Point, NY, Sean Kazmark of Manhattan, NY and Daniel Kazmark of San Diego, California.; and his siblings, David Charles Kazmark of East Patchogue, N. Y and Deborah Ann Kazmark of Long Eddy.
