He was predeceased by: his parents, Everett S. Cook Sr. and Minnie Cook (Palmitier); and his siblings, John Cook (D. Evelyn Cook), Ruth Clark and Sally Shilke.
He is survived by: his sister Beverly Sklanka (D. Joseph Sklanka); his mother-in-law Jane Kittle (D. Duane Kittle); his son David; and his grandchildren, Landon, Leona and Allison. He is also survived by step siblings, as well as many nieces and nephews.
