He was predeceased by: his parents, Edward Cook and Dorothy Cook. He is survived by: his wife Kathleen (Mullooly); his children, Jack (Jackie), Sheila (Matt), Michael (Frankie) and Kevin; and his grandchildren, Aidan, Liam, Codie and Patrick.

Visitation was held on Monday, December 16th 2024 from 2:00 PM to 4:00 PM and from 7:00 PM to 9:00 PM at the Macken Mortuary (52 Clinton Ave, Rockville Centre, NY 11570). A funeral service was held on Tuesday, December 17th 2024 from 10:45 AM to 11:30 AM at the St. Agnes Cathedral (29 Quealy Pl, Rockville Centre, NY 11570).
