He was predeceased by: his parents, Sherman Donnelly and Beatrice Donnelly; and his siblings, Doug, Debbie Johnson, Sherman and Guy.

He is survived by: his wife Suzanne "Sue" Donnelly; his sons, Michael and Brandon Donnelly; his siblings, Coleen, Tina (Linda) and David Donnelly (Phyllis); and his pets, Princess and Colby. He is also survived by many grandchildren, nieces and nephews.

Donations may be made in Perry's name to the Flower City Sportsmen, 35 Woodlawn Street, Rochester NY, 14607.
