He was predeceased by: his father Ronald C. Sample of Randolph; and his stepfather Mark L Andersen of Jamestown. He is survived by: his parent Darileen Rouse Andersen of Jamestown; his sister Kathy Sample; his significant other Wendy Keller; and his children, Sharayah Burrows, Jessie Anderson, David Waite, Breanna, Tasha, Shania, Sabrina, Austin and Brooke Sample.
