He was predeceased by: his parents, Thomas Zak and Nellie Reyman Zak; and his siblings, Edward Zak, Victoria Sroka, Pauline Bisanzo, Mary Marcin and Agnes Talar.

He is survived by: his wife Joan MacFarlane Zak; his daughters, Donna Zak of Port Chester and Laura Holman (David) of Los Angeles, CA.; his grandson Jake Holman; and his sister Phyllis Snedden of Boise. He is also survived by several nieces and nephews.

In lieu of flowers, memorial donations may be made to the Alzheimer's Association.
