He was predeceased by: his parents, Henry Blaise and Helen Blaise; his son David Blaise; and his brothers, Howard, Henry Jr. and Kenneth.
He is survived by: his wife Denise Blaise; his daughter Helen; his daughter-in-law Anna Blaise; his sisters, Sally Hemingway and Helen Desin; and his grandchildren, David, Joshua and Kendra. He is also survived by several great-grandchildren; and several nieces and nephews.
