He was predeceased by: his parents, Helen Cook and Robert Cook of Amenia, NY.; and his brother Gerry Cook. He is survived by: his son Jason R. Cook (Brian); his wife Judy Cook; his son Paul Cook (Lisa); his grandchildren, Aaron Cook, Ruth Cook, Claire Cook and Julia Cook; his siblings, Kevin Cook (New) and Judy Westfall (Doug); and his niece Alyssa Valerio (Michael).
