He is survived by: his wife Jean M. Evans; his children, Cynthia Evans (Gary Sanfilippo), Kim Evans (David), Scott Evans (Shannon) and Michael Evans (Lydia); his grandchildren, Megan Kloosterman, Jennifer Kloosterman, Elissa Evans, Serafina Rose Evans, Sawyer Gale O'Farrell, Gage Evans and Jace Evans; and his siblings, Ernie Sample (Suzette) and Ginger Sample. He is also survived by nieces, nephews, cousins & dear friends.
In lieu of flowers donations may be sent to St. Jude Children's Hospital, 501 St. Jude Place, Memphis, TN 38105 in Pat's memory.
