He was predeceased by: his parents, John R. Cook Sr. and Marguerite A. Thompson. He is survived by: his wife Angela M. Cook; his daughters, Skylar and Sienna Cook; his grandchild Braxtyn Mahnke; his parents-in-law, Kim and Kurtis Loud; his brothers-in-law, Joshua Loud (Heather Loud) and Dana Loud (Carrie Vooght); his cousin Carrie Unrue (Tony Unrue); his siblings, Jason Loscig (Traci Knapp) and Becky Wheaton (Mike Moore); and his nephew Christopher Brockway Jr..
