He was predeceased by: his parents, Suzanne M. Forsyth and Carl H. Forsyth; his cousin Shane A. Chipy; and his grandparents, Jim, Cecelia Green and Ethel Forsyth.
He is survived by: his son James S. Forsyth; his stepson Cory J. Stewart; the mother of his children Lynnae Spencer; his sister Amanda Holt (Noel Holt); his nieces and nephews, Nathan, Noah and Addison; his brother Christopher Forsyth (Janet Forsyth); his nephews, Michael and Gregory; and his aunts, Sarah Chipy, Ellie Green and Patricia Pickett.
