He was predeceased by: his parents, Chester Cook and Blanche Fox Cook; his siblings, Anita Landers, Gerald and Michael Cook; his nephews, Ryan Hughson and Nicholas Cummings; his parents-in-law, Betty Morrison Boughton (Bob) and Roger Morrison; and his brother-in-law Jimmy Stapleton.
He is survived by: his wife Jackie Cook; his sons, Jamie (Kelly), Jason (Caitlyn) and Jordan Cook; his siblings, Donna Hughson (Richard) and Charles Cook; his brother-in-law Richard Landers; his grandchildren, Madison of Andrew, J. Sideways, Jaxson "Spanky" and Jaiden "Junior"; his nephew Mark Hughson (Kristy); and his close friend Jamie Cosco. He is also survived by many nieces, nephews and extended family.
