He was predeceased by: his parents, Herbert Phillips, and Evelyn George Phillips, Sr.; his wife Carol Welch; and his daughters, Laurie Lee Phillips, Carol Lee Phillips and Lynn K. DuChene.
He is survived by: his sons, Daniel M. Phillips (Kim Upfold) and Melvin L. Phillips, Jr. (Lorraine Cook) of Oneida; his son-in-law John T. DuChene of Syracuse; his grandchildren, Adam DuChene (Ariel), Melvin L. Phillips III, Amanda Phillips and Lacie M. Burke (Garrett); and his great grandchildren, Isaac, Sam, Charliee, Sophie and Henry. He is also survived by brother and three sisters and several nieces, nephews and cousins.
