He was predeceased by: his wife Dorothy Johnston; his sons, Kevin and John; and his siblings, Mary, William and James.
He is survived by: his parents, George Forrest and Phyllis Forrest; and his children, Mary Mendola (Joseph) of Arizona, Karen Bassano (Bradley) of Lancaster and William of Erie. He is also survived by many grandchildren and great-grandchildren; and several nieces and nephews.
A memorial reception will be held from 4 to 7 p.m. Saturday (Dec. 22, 2018) at the Olean American Legion, 307 E. State St. In lieu of flowers, memorials may be made to the Penn York Golf League, c/o Kyle Henzel, 206 N. Fifth Street, Olean, NY 14760.
