He is survived by: his girlfriend Ellen Zastrow; his close friend Ellen Zastrow; his children, April Kubiniec (Joe), Jennifer O'Brien (Pat) and Steven Cook; his grandchildren, Kasey, Haleigh Kubiniec, Nicholas, Shane, Troy Wilkins, Christopher and Sawyer Cook; his siblings, Carol Wiswell, R. Jeanne Sax (Roy), Linda Clare (Roy) and Robert Cook; and his sister-in-law Virginia Cook.
