He was predeceased by: his parents, George Cook and Dorothy Cook (Blackman). He is survived by: his wife Mary; his sons, John Cook of Bakersfield, CA, David Cook of Poland and Scott Cook (Crystal) of Poland; his grandchildren, Corey and Rachael Cook; and his siblings, Lawrence Cook of Newport, Harold Cook of Poland, Jack Cook (Patricia) of North Bay and Eleanor Miller (Robert) of Remsen. He is also survived by many in-laws, nieces and nephews.
