He was predeceased by: his parents, Harry Forrest and Elsie Locke Forrest; and his siblings, Kenneth, Eleanore Kent and Shirley Forrest.

He is survived by: his wife Martha L. Vanderhoef Forrest; his children, Sharon Dodd (Timothy), Kathie Telehany (Michael), Harry Forrest (Amy) and Howard Forrest (Kim); and his siblings, Marilyn Forrest and William Forrest (Roberta). He is also survived by twenty grandchildren, five great grandchildren.
