He was predeceased by: his father William Maynard. He is survived by: his wife Kristina; his children, Samantha and Eli Maynard; his mother Sallie Moore-Rafferty; and his brothers, William Maynard II (Katherine), Michael Maynard (Lisa) and Adam Maynard (Shauna). He is also survived by many extended family including nieces and nephews.
