He was predeceased by: his parents, John J. Hayes and Ida Hayes (Deragon); his daughter Dawn Ida Hayes; his grandson Joshua Hayes; and his brothers, William Hayes and Joseph Hayes.
He is survived by: his wife Carole J. Hayes (Lovely); and his children, John G. "Jack" Hayes (Michaelena), Shawn Hayes, Kelly Doty (Ernie Mollnow) and Michael Hayes (Pamela). He is also survived by 11 grandchildren and 8 great grandchildren.
