He is survived by: his wife Cheri Phelps Bartok; his daughters, Bethany Gonzalez of Jamestown and Madison Bartok (Alex Bartok); his sister Tami Cucinotta of Mayville; his nieces and nephews, Joshua Bartok (Kate), Adam, Christopher Winne, Meghan, Allison Bartok and Jordyn Cucinotta; his great-nephews, Jacob and Embersen; his great-niece Claire; his uncles, Robert and Philip Roche of Greenhurst; and his aunt Linda White of Seven Hills, OH. He is also survived by several other aunts, uncles and cousins.
